1. Scope of Services
Solid Foundations RN Care, PLLC provides non-emergent, supportive nursing and nail care services delivered by a licensed Registered Nurse (RN) in accordance with the Texas Board of Nursing Rules and Scope of Practice.
Our services are designed to promote safety, comfort, education, and wellness within the RN’s legal scope.
We do not:
- Diagnose medical or podiatric conditions
- Prescribe medications
- Initiate or alter medical or podiatric treatment plans
- Replace the care of a physician, podiatrist, or other licensed provider
Clients are expected to maintain ongoing care with their medical providers and follow up promptly on any recommendations.
2. Emergency and Urgent Care Limitations
Solid Foundations RN Care, PLLC does not provide emergency or urgent care.
If you experience a life-threatening or urgent condition, you must call 911 or seek emergency medical attention.
3. Payment and Financial Policy
- Services are private pay only.
- We do not bill Medicare, Medicaid, private insurance, or managed-care plans.
- Payment is due at the time of service unless otherwise arranged.
- Accepted forms of payment include: cash, personal check, debit/credit cards, Apple Pay, Venmo, and Cash App.
- HSA/FSA cards may be used at the client’s discretion; clients are solely responsible for confirming reimbursement eligibility.
- Travel fees may apply depending on mileage and will be disclosed prior to scheduling.
Failure to pay for services may result in discontinuation of care.
4. Client Rights
Clients have the right to:
- Respectful, non-discriminatory care
- Ask questions and receive explanations about services
- Decline any service offered
- Confidential handling of personal health information in accordance with state and federal laws
5. Client Responsibilities
Clients (or their representatives) agree to:
- Provide accurate, complete, and up-to-date health information
- Disclose all known medical conditions, medications, allergies, and changes in health status
- Maintain an established relationship with their medical providers
- Follow all medical recommendations provided by their physicians or podiatrists
- Ensure a safe, clean, and respectful environment during home visits
The Company reserves the right to discontinue or refuse services if the home environment is unsafe, unsanitary, hostile, or inappropriate.
6. Limitations of Services & Outcomes
All services provided are supportive and educational in nature. Solid Foundations RN Care, PLLC does not guarantee specific results or outcomes, and services should not delay or replace necessary medical or podiatric care.
To the fullest extent permitted by law, the Company is not liable for damages related to:
- Preexisting health conditions
- Delay in seeking medical treatment
- Incomplete or inaccurate information provided by the client
- Use or misuse of services
- Client’s failure to follow medical advice from licensed providers
7. Acknowledgment of Care Provided
By receiving services, you acknowledge that:
- Care is provided strictly within the Registered Nurse Scope of Practice in Texas
- You remain responsible for following up with medical providers
- You understand the purpose and limitations of non-emergent, private-pay RN services
- You have provided health information to the best of your knowledge
A patient-provider relationship is not established until the intake process is completed and the Company confirms acceptance for services.
