Medication Management     

Biweekly medication organization by a Registered Nurse to ensure safe, accurate dosing. All we need is your most up to date medication list, your prescription medications in bottles, pill organizer and we will prepare you from there! Includes refill coordination with your pharmacy as needed, adherence monitoring, and education on medication storage, purpose, timing, and possible side effects. This service helps reduce medication errors and supports better health outcomes. $85 per visit. 

We can provide a pill planner/organizer for an additional fee upon request.
