Solid Foundations RN Care, PLLC (“we,” “our,” or “us”) is committed to protecting the privacy and confidentiality of client health information. This Privacy Policy explains how we collect, use, disclose, and safeguard your personal and protected health information (PHI) in compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA), the Texas Medical Records Privacy Act (TMPA) (Texas Health & Safety Code, Chapter 181), and other applicable federal and state laws.

- Identifying details (name, date of birth, contact information)

- Medical history, diagnoses, and treatment plans

- Images obtained for clinical documentation

- Medications and allergies

- Physician orders, nursing assessments, and clinical notes Images obtained

- Payment and billing information (if applicable)

Use of Information

Your information may be used for the following purposes:

- To provide safe, appropriate nursing care within the RN scope of practice

- To coordinate and communicate with your healthcare providers, caregivers, or authorized family members

- To comply with documentation, billing, and legal requirements

- To improve care quality and safety

Use of Images and Photographs

Solid Foundations RN Care does not capture, store, or share client photographs or videos without prior written consent. Any client images obtained for clinical documentation, education, or marketing purposes (including before and after images of nail care) will be used only with written permission and will be deidentified to protect client privacy. Clients may decline photo use at any time without affecting their ability to receive services.

Disclosure of Information

We will only share your information as permitted by law:

- With your written authorization for purposes beyond treatment, payment, or healthcare operations

- Without authorization when required by law (e.g., public health reporting, suspected abuse or neglect, responding to legal proceedings, or serious threats to health or safety)

- With healthcare providers directly involved in your care, when necessary for treatment or emergency response

- To business associates (such as billing services) under written agreements requiring HIPAA compliance

Safeguards and Protection

We maintain administrative, technical, and physical safeguards to protect your PHI, including:

- Secure storage of paper and electronic records

- Password-protected devices and encrypted communication where possible

- Limited access to client records based on role and necessity

- Staff and contractors trained on HIPAA compliance and confidentiality obligations

Your Rights as a Client

Under HIPAA and Texas law, you have the right to:

- Access and obtain copies of your medical records

- Request corrections to inaccurate or incomplete information

- Restrict disclosures of your PHI, where legally permissible

- Request confidential communications through alternate contact methods

- Withdraw prior consents or authorizations in writing (except where already relied upon)

- File a complaint without fear of retaliation if you believe your privacy rights have been violated

Retention of Records

Medical records will be maintained for the period required under Texas law (minimum of 7 years for adults; until age 21 for minors), after which they will be securely destroyed.

We reserve the right to revise this Privacy Policy at any time as required by law or business practices. The most current version will be made available upon request.
